ASSUMPTION OF RISK AND RELEASE OF CLAIMS
I acknowledge and understand that participation in dance classes, workshops, rehearsals, or performances may involve inherent risks, including, but not limited to, the risk of physical injury or death. By signing this agreement, I voluntarily assume full responsibility for any injury, harm, or accident that may occur to me or my child during any activity organized by Young Reach Dance Company LLC.
I hereby release and discharge Young Reach Dance Company LLC, its owners, instructors, staff, assistants, volunteers, guest artists, and participants from any and all claims, demands, or legal actions arising from personal injury, death, or damage to personal property incurred while participating in studio activities.
I also voluntarily waive my rights, as well as those of my heirs or assigns, to pursue legal action against Young Reach Dance Studio, its employees, agents, or representatives for any incidents that may occur during participation in any classes or activities.
PHOTO AND VIDEO RELEASE
I understand that Young Reach Dance Company LLC may take photographs or videos during classes, workshops, performances, and other activities for promotional and educational purposes. By signing below, I grant Young Reach Dance Company LLC and its representatives permission to use photographs, videos, or other media that include my image or that of my child for use in advertising, marketing, social media, and other promotional platforms.
I agree that I will not receive any compensation or royalties for the use of such images or media, and I waive any right to inspect or approve the finished media or any printed or electronic materials that may use these images.
